1980s Forest Fancies Pyrex mixing bowl #401 (750 ml).  Measures about 5.5 inches across by about 3.5 inches high.  Good condition with one small mark on one of the mushrooms (see photo).

The story: My friend Anne got a set of these mixing bowls for her wedding in the mid 1980s.  It is the only one that survived, including that marriage.  Good news - I like her second husband better. :)
